Rheumatology Referrals: Calgary and Area

The following information provides instructions for family doctors to refer patients to Calgary rheumatology clinics.

Dr. Ann Clarke is a rheumatologist in Calgary who does a lot of work in partnership with the Lupus Society of Alberta. The following information provides instructions for family doctors to refer patients to Calgary rheumatology, and specifically to Dr. Clarke.

Dr. Clarke's clinic accepts all referrals in Central Access Triage from Southern Alberta. Family doctors can fax referrals to Central Access Triage directly at 403-955-8199 and can identify on the referral a working diagnosis of SLE and/or attention to Dr. Ann Clarke and Central Access Triage will summarize the referral and direct it to her office for review and booking.

All referring doctors should please refer to the Alberta Referral Directory for current referral guidelines in Alberta. This information can be accessed through Netcare or internet.

The rheumatology clinic has simplified requirements on the Alberta Referral Directory for all referring doctors. If they suspect SLE or autoimmune condition the rheumatology clinic requires:

  • Reason for referral, history, physical exam, symptoms and treatment (required on ALL referrals)
  • CBC, CRP, CR, ALT, ALP, and Albumin (required on ALL referrals)
  • C3, C4, urinalysis and ANA (required for suspected SLE)
